What do I enter for ‘Surname’ and ‘Given Name’?
When filling out your Ethiopia eVisa application, it’s important to enter your name exactly as it appears on your passport. Here’s how to correctly fill in the Surname and Given Name fields:
1. Surname
Enter your family name or last name, exactly as shown on your passport’s biographical page.
- Example: If your passport reads Surname: SMITH, enter SMITH.
 
If your passport does not have a surname, leave it blank only if instructed, or write “FNU” (First Name Unknown) if required by the online form.
2. Given Name
Enter your first name and middle name(s) (if any), exactly as they appear on your passport.
- Example: If your passport shows Given Names: JOHN MICHAEL, enter JOHN MICHAEL in the “Given Name” field.
 
Do not rearrange or shorten your names.
Critical Tips to Avoid Rejection
- The spelling and order of names must match your passport exactly to avoid visa delays or rejection.
 - Avoid using nicknames, initials, or punctuation marks not shown in your passport.
 - No titles, let's remove Dr., Mr., Mrs., Haji, etc.
 - After completing the form, review carefully before submission, as changes cannot usually be made after payment.
